Dospital does not collect any personal information of any individual unless the individual chooses to provide Dospital with that information or the individual voluntarily chooses a service where gathering personal information is necessary to deliver such specific service.

When collection of personal information is necessary for rendering a service, Dospital shall collect such information to a reasonably necessary extent.

To the extent reasonably necessary for rendering a service to an individual who voluntarily chooses Dospital as the service provider, Dospital may require proof of such individual’s identity
